- Personal Information – verify spelling and dates.
- Injury Details – include exact time, activity, and equipment involved.
- Medical Treatment – list all providers and services received.
- Signature Block – employee and employer signatures, with dates.
Common Mistakes to Avoid
Missing or incorrect data can delay processing:
- Omitting the employee's Social Security number.
- Failing to attach proof of medical treatment.
- Submitting a claim after the 30‑day reporting window.
Submitting the Forms
After completion:
- Print all pages and sign where required.
- Scan or photograph each page to create a PDF bundle.
- Upload the bundle to the PGCPS workers' compensation portal or email it to the HR department at hr@pgcps.org.
Tracking Your Claim
Once submitted, the portal assigns a case number. Log in to view status updates, requested additional documents, and approval notices. If no update appears within 10 business days, contact the claims officer listed on the site.
