Finding the Forms

The Public Garden City Public Schools (PGCPS) website hosts a dedicated workers' compensation portal. Navigate to the main site, click the "Human Resources" tab, and select "Workers' Compensation." A PDF download link appears for each required form: Claim Form, Medical Release, and Employer Notification.

Preparing Your Information

Before opening the PDF, gather the following:

• Employee's full name, address, and contact details.
• Employer identification number and supervisor's name.
• Date and location of the injury, and a brief description of the incident.
• Medical provider information and any preliminary treatment notes.

Filling Out the Claim Form

Open the Claim Form PDF. Fields are clearly labeled; use a black or blue ink pen if printing, or fill electronically with a PDF editor. Key sections:

• Personal Information – verify spelling and dates.
• Injury Details – include exact time, activity, and equipment involved.
• Medical Treatment – list all providers and services received.
• Signature Block – employee and employer signatures, with dates.

Common Mistakes to Avoid

Missing or incorrect data can delay processing:

• Omitting the employee's Social Security number.
• Failing to attach proof of medical treatment.
• Submitting a claim after the 30‑day reporting window.

Submitting the Forms

After completion:

• Print all pages and sign where required.
• Scan or photograph each page to create a PDF bundle.
• Upload the bundle to the PGCPS workers' compensation portal or email it to the HR department at hr@pgcps.org.

Tracking Your Claim

Once submitted, the portal assigns a case number. Log in to view status updates, requested additional documents, and approval notices. If no update appears within 10 business days, contact the claims officer listed on the site.
